GO2Health’s Coordinated Veteran Care (CVC) system is our comprehensive care administration program geared towards preventing chronic infection in veterans.
Most veterans we treat are aged 25 -55 and our CVC was created as a program that is holistic of medication. We discuss your quality of life goals with you, and from that creates your вЂGreen Folder’ – your comprehensive medical record.
In your Green Folder we record such things as:
- Medicines you are taking
- Results from any testing tests, like bloodstream tests, epidermis checks, mental health assessments
- Results of any checks that are biomechanical like those carried out by a physiotherapist or podiatrist
- Conversations we now have about building a wholesome life style – including restricting medication and liquor usage, increasing workout and exercising mindfulness
- Whom accocunts for your support system, including family members relationships
- Treatment plans developed they complement other treatment plans for you by the allied health professionals at GO2 Health, ensuring
- Other things that provides us and you also an overview that is complete of health and wellness
